Three Academy graduates are drafted into the starting line-up as Bristol go in search of a victory that would seal promotion at Sixways (7.45pm kick-off).
Jack Tovey starts in the midfield, while Marco Mama and Ben Glynn come into the pack for the Championship final second leg.
Andy Robinson makes a total of five changes from the side that suffered a 28-29 loss in the first leg, as Ben Mosses and Nicky Robinson are also named in the backline.
Ryan Jones (shoulder) and Gavin Henson (shin) are unavailable for selection.
Olly Robinson comes onto the replacements bench as Bristol retain a four forwards and three backs split.
